f658d7daba [r8169] Remove driver cfg lookup, use pci_device_id->driver_data instead
This patch removes the cfg lookup made in the r8169 driver and
replaces it with equivalent information found in the driver_data field
of the pci_device_id structure.

8406115834 [build] Rename gPXE to iPXE
Access to the gpxe.org and etherboot.org domains and associated
resources has been revoked by the registrant of the domain.  Work
around this problem by renaming project from gPXE to iPXE, and
updating URLs to match.

Also update README, LOG and COPYRIGHTS to remove obsolete information.

37a0aab4ff [netdevice] Separate out the concept of hardware and link-layer addresses
The hardware address is an intrinsic property of the hardware, while
the link-layer address can be changed at runtime.  This separation is
exposed via APIs such as PXE and EFI, but is currently elided by gPXE.

Expose the hardware and link-layer addresses as separate properties
within a net device.  Drivers should now fill in hw_addr, which will
be used to initialise ll_addr at the time of calling
register_netdev().

b59e0cc56e [i386] Change [u]int32_t to [unsigned] int, rather than [unsigned] long
This brings us in to line with Linux definitions, and also simplifies
adding x86_64 support since both platforms have 2-byte shorts, 4-byte
ints and 8-byte long longs.

1a867bfb39 Remove *_fill_nic() calls, and directly set nic->ioaddr and nic->irqno .
This needs to be done manually because if the irq() routine is
implemented then we want something like "nic->irqno = pci->irqno;",
else we do "nic->irqno = 0;" nic->ioaddr may also need to be set
carefully.

Also added local variables to end of many files, for emacs indentation
to match kernel style (tab does 8 space indent).

dad5274522 Add "name" field to struct device to allow human-readable hardware device
names.

Add "dev" pointer in struct net_device to tie network interfaces back to a
hardware device.

Force natural alignment of data types in __table() macros.  This seems to
prevent gcc from taking the unilateral decision to occasionally increase
their alignment (which screws up the table packing).

15ee09ed10 Restructured PCI subsystem to fit the new device model.
Generic PCI code now handles 64-bit BARs correctly when setting
"membase"; drivers should need to call pci_bar_start() only if they want
to use BARs other than the first memory or I/O BAR.

Split rarely-used PCI functions out into pciextra.c.

Core PCI code is now 662 bytes (down from 1308 bytes in Etherboot 5.4).
284 bytes of this saving comes from the pci/pciextra split.

Cosmetic changes to lots of drivers (e.g. vendor_id->vendor in order to
match the names used in Linux).

824d6ffa7f Header rearrangement.
I want to get to the point where any header in include/ reflects a
standard user-level header (e.g. a POSIX header), while everything that's
specific to gPXE lives in include/gpxe/.  Headers that reflect a Linux
header (e.g. if_ether.h) should also be in include/gpxe/, with the same
name as the Linux header and, preferably, the same names used for the
definitions.
