82ecaaac91 [console] Remove "log message" usage from interactive console defaults

e024cd39a8 [console] Allow usage to be defined independently for each console
Add the concept of a "console usage", such as "standard output" or
"debug messages".  Allow usages to be associated with each console
independently.  For example, to send debugging output via the serial
port, while preventing it from appearing on the local console:

  #define CONSOLE_SERIAL CONSOLE_USAGE_ALL
  #define CONSOLE_PCBIOS ( CONSOLE_USAGE_ALL & ~CONSOLE_USAGE_DEBUG )

If no usages are explicitly specified, then a default set of usages
will be applied.  For example:

  #define CONSOLE_SERIAL

will have the same affect as

  #define CONSOLE_SERIAL CONSOLE_USAGE_ALL

8406115834 [build] Rename gPXE to iPXE
Access to the gpxe.org and etherboot.org domains and associated
resources has been revoked by the registrant of the domain.  Work
around this problem by renaming project from gPXE to iPXE, and
updating URLs to match.

Also update README, LOG and COPYRIGHTS to remove obsolete information.

3f85626fa9 [efi] Add efi_strerror()
EFI_STATUS is defined as an INTN, which maps to UINT32 (i.e. unsigned
int) on i386 and UINT64 (i.e. unsigned long) on x86_64.  This would
require a cast each time the error status is printed.

Add efi_strerror() to avoid this ickiness and simultaneously enable
prettier reporting of EFI status codes.

b59e0cc56e [i386] Change [u]int32_t to [unsigned] int, rather than [unsigned] long
This brings us in to line with Linux definitions, and also simplifies
adding x86_64 support since both platforms have 2-byte shorts, 4-byte
ints and 8-byte long longs.

81d92c6d34 [efi] Add EFI image format and basic runtime environment
We have EFI APIs for CPU I/O, PCI I/O, timers, console I/O, user
access and user memory allocation.

EFI executables are created using the vanilla GNU toolchain, with the
EXE header handcrafted in assembly and relocations generated by a
custom efilink utility.
